Reusing github readme with link to other markdown

I’d like to reuse my existing Readme.md in the root of my repo as an ExDoc page. However it links to another markdown file in the same repo. Is it possible to make this link work both on the github web interface and in the exdoc generated html pages? The problem is that exdoc needs to be so smart to rewrite this link from an .md file to an .html file of the same name. (Or is it possible to run a custom script before exdoc to do the replacement?)
